This subproject is one of many research subprojects utilizing theresources provided by a Center grant funded by NIH/NCRR. The institution listed isfor the Center, which is not necessarily the institution for the investigator.Blood pressure (BP) is currently controlled in only 27% of the population. Poor BP control exists in spite of six sets of guidelines generated over the past 30 years in the U.S. The objective of this study is to test whether BP control can be improved by physician education and feedback provided through the development of physician/pharmacist collaborative teams. The primary hypothesis of this study is that BP's will be reduced to a greater extent in patients who receive care from physican/pharmacist teams when compared to those who receive care from physicians alone.
