The institution listed isfor the Center, which is not necessarily the institution for the investigator.The objective of this study is to evaluate the response, as determined by resolution of vulvar pain symptoms, following oral gabapentin in a double blind fashion, in women with vulvodynia. Despite the large number of women affected, the natural history, etiology, and effective treatment of vulvar pain syndrome remains poorly understood. Specifically, the epidemiology and thus, optimal treatment of vulvar pain syndrome including vulvodynia and vulvar vestibulitis syndrome (VVS) remain elusive. Numerous descriptive studies have attempted to identify possible etiologies of vulvodynia and VVS without consensus. Proposed mechanisms include: infectious, autoimmune, altered pH, irritants, idiopathic causes, as well as other less frequently explored mechanisms. Additionally, vulvodynia has been hypothesized to be a neurogenic pain syndrome. These investigators propose that vulvodynia is a neuropathic pain and reflex sympathetic dystrophy disorder as characterized by allodynia, hyperalgesia, and burning pain. As such, treatment must be directed toward the suppression of the neurostimulatory response. Gabapentin has been shown to be effective in treating chronic pain syndromes, including neuropathic pain and reflex sympathetic dystrophy. Anecdotal evidence including case reports support the use of gabapentin in the treatment of vulvodynia including VVS.
