The purpose of this study is to evaluate the ability of revascularization (surgical procedures leading to improved blood flow), such as coronary artery bypass graft (CABG) surgery, to increase survival in patients with CHF as compared to medical therapy alone. The study will also evaluate if changing the shape and size of the heart by surgical ventricular restoration (SVR) when added to CABG will further improve survival. The study will also look at the relationship of treatment received and the heart's function before and after the treatment
