This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The overall goal of this project is to determine which girls with premature pubarche are at risk of developing PCOS. Three hypotheses will be tested. Hypothesis 1: The 17-hydroxyprogesterone (17HPROG) response to a human chorionic gonadotropin (hCG) test of ovarian function will be higher after gonadotropin releasing hormone agonist (GnRHag) than alone. Hypothesis 2: The magnitude of the 17HPROG response to hCG will correlate with the number of ovarian follicles on ultrasound, the 17-hydroxypregnenolone (17HPREG) response to ACTH, and indexes of insulin resistance. Hypothesis 3: The highest quartile of testosterone levels will be found in patients homozygous for a candidate activating variant ('G-variant') of the 17 -hydroxysteroid dehydrogenase type 5 gene. To test these hypotheses, 6-9 year old girls with premature pubarche will undergo glucose tolerance, cosyntropin, gonadotropin releasing hormone agonist (leuprolide), and human chorionic gonadotropin testing and a pelvic ultrasound examination.
