The institution listed isfor the Center, which is not necessarily the institution for the investigator.The objective of this study is to compare health and economic outcomes between patients with coronary heard disease (CHD) who are randomized to routine care or care provided by the Cardiovascular Risk Reduction Clinic (CRRC) in addition to routine care. The primary endpoint of study will be change in low-density lipids (LDL) between groups. Secondary endpoints will compare groups with regard to a variety of clinical, humanistic, and economic variables.
