Project Summary: This is pilot study to evaluate the role of exercise in regulation of coronary artery blood flow. Although studies in animals have shown that sympathetically mediated coronary vasoconstriction occurs during static contraction, there is little data evaluating coronary flow response to exercise in human subjects. Historically the need for invasive flow monitoring has made it difficult to study real-time coronary hemodynamics during exercise. Recent studies using non-invasive trans-thoracic Doppler ultrasound demonstrated similar flow properties in LIMA (left internal mammary artery) bypass grafts and coronary arteries, suggesting this approach may allow non-invasive monitoring of coronary hemodynamics. The investigators propose to use this technique in 15 subjects that have undergone LIMA bypass grafting within the past 5 years to study the effect of handgrip exercise on coronary artery flow. Exclusion criteria are designed to exclude subjects with cardiac disease or co-morbidities that may alter coronary flow. A dobutamine stress echo will be performed to exclude subjects with significant coronary ischemia. Real-time ultrasound Doppler measurements of mean blood velocity will be performed in the LIMA left internal mammary artery graft and renal arteries during voluntary and involuntary handgrip exercise, and during handgrip and post-handgrip ischemia. Venous norepinephrine (NE) levels will be measured at baseline, and following the exercise paradigms (4 samples/subjects).
