The Section of Reproductive Endocrinology, Department of Obstetrics and Gynecology at the University of Colorado Health Sciences Center is uniquely positioned to participate as a Reproductive Medicine Unit in the Cooperative Multicenter Reproductive Medicine Network (RMN). The Section's clinical activities are focused at the Center for Reproductive Medicine at the University of Colorado Health Sciences Center, but full-time faculty members are located at three different physical sites, each of which can channel patients to clinical studies performed under the umbrella of the RMN. The Center for Reproductive Medicine is unique in its own way in that it is not housed in a single department. Rather, faculty in reproductive endocrinology, general gynecology, urology and psychology all see patients at the same site in order to promote optimal clinical cooperation in the care of both simple and complex reproductive disorders. Our application is buttressed by significant support in the reproductive medicine (reproductive endocrinology, urology) community for this endeavor. Finally, there is committed Departmental and Institutional support for the establishment of a network site at the University of Colorado Health Sciences Center. This application will describe existing organizational infrastructure, key personnel, professional relationships, and clinical base which will support the University of Colorado as a network site. We will also describe developments in progress which enhance our ability to contribute to the NIH Reproductive Medicine Network. Finally, we will present a """"""""concept"""""""" protocol that will reflect the ideas and capabilities of the investigators. We propose a randomized controlled trial to evaluate the overall impact of embryo transfer on day 3 compared with day 5 in an unselected IVF population. We hypothesize a strategy of """"""""maximal embryo usage,"""""""" with a combination of fresh and frozen embryo transfer, will optimize pregnancy rates, minimize multiple gestations and maximize cost efficiency both for individual couples and the public sector. This research proposal will include documentation of the ability and experience of our investigators to participate in the development and design of clinical protocols and to participate in cooperative, multicenter trials.
