This subproject is one of many research subprojects utilizing theresources provided by a Center grant funded by NIH/NCRR. The subproject andinvestigator (PI) may have received primary funding from another NIH source,and thus could be represented in other CRISP entries. The institution listed isfor the Center, which is not necessarily the institution for the investigator.The Grady Interpregnancy Care (IPC) Project enrolls women who have recently delivered a VLBW baby (still-born or live-born) at Grady Hospital and provide them with 24 months of funded comprehensive, integrated, primary health care services, and enhanced case management. The purpose of this intervention is three-fold: (1) to improve the 'mother's' interpregnancy health and reduce her identified medical and social risks; (2) to assist the 'mother' in achieving her reproductive goals that may include a planned pregnancy with an interpregnancy interval of at least 9 months, but preferably 18 months; (3) to improve the 'mother's' future reproductive outcomes (in terms of increasing birth weight and/or gestational age of subsequent pregnancies).
