This subproject is one of many research subprojects utilizing theresources provided by a Center grant funded by NIH/NCRR. The subproject andinvestigator (PI) may have received primary funding from another NIH source,and thus could be represented in other CRISP entries. The institution listed isfor the Center, which is not necessarily the institution for the investigator.ACCORD is a multi-center study that will enroll 10,000 individuals with type-2 diabetes mellitus and either hypertension or hyperlipidemia who have cardiovascular (CV) disease and/or >2 CV risk factors. Subjects will be randomized to either standard (median HbA1c of 7.5%) or intensive (median HbA1c<6.0%) glycemic control. In addition one 2x2 trial of 5800 participants will evaluate the addition of fibrate therapy to statin therapy. A second 2x2 trial of 4200 patients will evaluate intensive (SBP<120 mmHg) versus conventional (SBP<140mmHg) hypertension therapy. Subjects will be followed for 4-8 years, with the primary outcome being the first occurrence of a major CV event, especially non fatal MI, non fatal stroke or CV death. The development or progression of diabetic retinopathy will also be evaluated
