The institution listed isfor the Center, which is not necessarily the institution for the investigator.Neonatal candidiasis is a lethal disease that is poorly understood. Despite an attributable mortality of 30%, the current gold standard for the diagnosis of neonatal candidiasis (repeated culture of normally sterile body fluid) has a sensitivity of 50%; and there have been no propsective studies to establish guidelines for the initiation of empirical treatment. This protocol will establish a prospective cohort of 1,750 infants born weighing less than 1000 grams in order to evaluate three new diagnostic tests and develop a risk stratification model for invasive candidiasis.
