This subproject is one of many research subprojects utilizing theresources provided by a Center grant funded by NIH/NCRR. The institution listed isfor the Center, which is not necessarily the institution for the investigator.To determine whether communication of a preventive nursing care plan between ambulatory and hospital nurses will reduce adverse events during hospitalization for older adults. Communication of nursing knowledge between ambulatory and hospital nurses during home-to-hospital transitions will focus on risk recognition that may be key to prevention and thus central to patient outcomes during hospitalization. This study will also identifying how and when nursing interventions impact nurse sensitive patient outcomes.
