This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The main hypothesis is that the circadian rhythms of 'intact' PTH, PTH-(1-84), PTH-(7-84), PTH(1-84)/PTH(7-84) ratio, and markers of bone formation and resorption are blunted in patients with CKD-5. This blunting is more pronounced in African American than in Caucasian patients and in diabetic than in non diabetic patients. Vitamin D treatment affects the circadian rhythms of PTH and bone markers. Specfic aims are to optain data on the above mentioned ratio. Compare these circadian rhythms of African American vs. Caucasian with CKD-5. To compare these circadian rhythms of diabetic vs. non diabetic patients with CKD-5. To compare these circadian rhythms in patients with CKD-5 on and off vitamin D treatment.
