This subproject is one of many research subprojects utilizing theresources provided by a Center grant funded by NIH/NCRR. The subproject andinvestigator (PI) may have received primary funding from another NIH source,and thus could be represented in other CRISP entries. The institution listed isfor the Center, which is not necessarily the institution for the investigator.The main objective is to determine what portion of patients presenting to the emergency department (ED) with acute illness and elevated blood sugar have undiagnosed diabetes or impaired glucose metabolism. By evaluating patients with both normal and elevated blood sugars, we will then determine a threshold that can be used to identify risk of having diabetes. The second objective is to determine whether the HbA1c in combination with the ED random glucose level enhances the clinician's ability to differentiate diabetes or impaired glucose metabolism from 'stress hyperglycemia'. We will also determine whether combining this information with other easily obtained variables (e.g. body mass index, age, past medical history) increases the precision of risk stratification for diabetes or impaired glucose tolerance.
